Job Summary As a Wound Care Nurse Practitioner, you will implement policies and procedures along with established care standards to manage patient needs related to various wounds. You'll provide direct patient care assistance to nursing staff and serve as a consultative resource regarding wound care practices, all under the oversight of a Wound Care Coordinator. Essential Functions Deliver specialized care for patients with wound-related needs. Thoroughly evaluate wounds upon admission and throughout care to formulate effective treatment plans and identify problems early. Collaborate with the supervising Registered Nurse and Wound Care Coordinator on initial treatment plans. Communicate any changes in wound status promptly to the supervising Registered Nurse and Wound Care Coordinator. Assist in planning and implementing strategies for wound treatment and prevention. Assess products for effectiveness and cost-efficiency during hospital usage and make recommendations accordingly. Participate in the development of patient discharge plans. Educate nursing staff on wound care products available within the hospital. Analyze culture results, track wound progression, and work alongside physicians and Wound Care Coordinators on treatment strategies. Contribute to root cause analysis and facilitate CMS quality data reporting as needed. Knowledge/Skills/Abilities/Expectations Adaptability to new situations and ability to prioritize effectively using problem-solving skills. Strong knowledge of wound care principles. Competence in wound debridement as per the defined level of practice. Resourceful in providing guidance to nursing staff on complex wound management. Capability to lead and motivate individuals and teams. Ability to collaborate and work cooperatively within a team environment. Willingness to perform other related duties as assigned. Qualifications Education Graduate from an accredited nursing program that leads to RN licensure. Licenses/Certifications Current RN license. BLS certification. Experience A minimum of three (3) years of nursing experience, with a focus on acute care.
