The Patient Navigator is a registered nurse dedicated to guiding and supporting women and their families through pre-pregnancy, pregnancy, and postpartum care by serving as a liaison, advocate, and educator to ensure seamless, high-quality, continuous care. They utilize their knowledge of women's health to address specific concerns and needs, fostering effective communication and customer service while resolving issues promptly to enhance patient experience. This role requires strong coordination skills, a compassionate approach, and the ability to navigate complex healthcare systems, ultimately promoting optimal health outcomes and positive patient relationships. Requirements: 32 hours per week - First Shift Monday - Friday 8:00 AM - 5:00 PM Valid RN License in the State of Indiana BLS/CPR Certification (3) Years of Clinical Nursing Experience Related to Obstetric or Pediatric Care Experience in Community Nursing or Care Coordination
