This role includes supervising Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees, in a supportive and safety-focused environment.**Key Responsibilities:**- **Patient Assessment & Care Planning:** Perform comprehensive nursing assessments on all patients within 8 hours of admission, forming the basis of individualized care plans. Identify teaching needs and collaborate with patients, families, and interdisciplinary teams to establish care goals. - **Documentation:** Ensure accurate and timely documentation of all patient assessments, interventions, and responses in the medical record, adhering to organizational policies. - **Patient Rehabilitation:** Participate actively in patient rehabilitation programs, prioritizing education for patients and their families based on identified health care needs. - **Discharge Planning:** Initiate discharge planning at the time of admission and begin educating patients about their discharge needs early in their stay. - **Communication:** Maintain clear communication regarding changes in patient status among care providers to support continuity of care and patient safety. - **Supervision & Teamwork:** Provide direction, training, and supervision to ancillary nursing personnel, employing effective decision-making, organization, and time management skills.
