**Position Overview:**

• **Weekends/Holidays:** As required

**Requirements:**
• **License:** Active RN
• **Certifications:** BLS
• **Experience:** 2+ years in home health
• **Skills:** Proficient in HCHB, OASIS, and Oasis E (post•Jan 2023)
• **Productivity:** 26•28 points/week
• **Case Load:** 20•25 cases/week
**Key Responsibilities:**
• Deliver quality home health care
• Complete same•day documentation
• Attend weekly in•person meetings
• Adapt to patient needs with flexible scheduling
