Role Overview Focus: Inpatient Discharge Planning (NOT Utilization Review) Primary Units: Medical-Surgical (MS) or Telemetry (Tele) Collaborates with: Social Worker (SW) , Physicians , and Interdisciplinary Team Expected Duties: Rounding with physicians/team Coordinating referrals (e.g., SNF, housing, hemodialysis, wound care/KCI) Discharge planning to home, community, or facility Managing complex social needs (e.g., homelessness support)

Must be: Independent Critical thinker Organized with strong time management Team-oriented with a positive attitude

Caseload & Coverage Typical floor census: 33–40 patients Staffing: Standard: 1 CM + 1 SW → Caseload: 17–25 patients Pilot Floors: 1 CM + 2 SWs → Caseload: 12–17 patients

Minimum expectation : Ability to handle at least 20 patients Common coverage: Internal Medicine and Family Medicine patients Systems & Tools EMR : Allscripts Preferred Experience With : InterQual MCG (Milliman Care Guidelines) CarePort

Utilization Review : Handled by a separate department Scheduling Hours : Monday–Friday, 8:00 AM–4:30 PM (7.5-hour shift + 1-hour lunch break) Management encourages taking a full lunch

Weekends & Holidays : Rare, manager approval required May involve covering multiple units

Dress Code Business Casual or Any Color Scrubs Except: No Royal Blue or Maroon Preferred: Neutral colors
