Seeking a Case Manager RN to coordinate patient care across the healthcare continuum. The ideal candidate will use evidence-based practice to evaluate appropriateness of care, monitor transitions, collaborate with interdisciplinary teams, and ensure compliance with regulatory standards. Key Responsibilities: • Evaluate utilization of resources, LOS, discharge readiness • Coordinate care and transitions for complex medical/surgical patients • Apply CMS, Title 22, and TJC guidelines • Serve as liaison between patient, provider, and insurance • Advocate for safe discharge planning and post-acute placement Schedule: • Day Shift: 5x8 (08:00–16:30) • Every 3rd weekend required • No on-call Ax
