Job responsibilities:
- Perform medication reconciliation activities as directed and communicate discrepancies.
- Communicate clinically relevant updates to providers to support timely decision-making.
- Work 40 hours per week with 8-hour shifts.
- Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams.
- Administer ordered therapies and monitor for adverse reactions or changes in condition.
