Job responsibilities:

Support care coordination activities across providers to promote continuity of care.

Perform medication reconciliation activities as directed and communicate discrepancies.

Work 40 hours per week with 8-hour shifts.

Use infection prevention practices during patient encounters and procedures.

Conduct patient intake, focused assessments, and symptom screening aligned with clinic protocols.
