RESPONSIBILITIES:
The RN Clinical Care Delivery Program Manager plans and provides comprehensive PACT training for the entire staff in accordance with professional nursing standards. Additionally, the RN Program Manager actively monitors clinical quality metrics as listed in the QASP having to do directly with medical care delivered and outcomes. Education and improvement plans are designed, implemented and monitored for outcomes. Program manager will be required to travel as needed monthly to train new staff, implement new processes, cover Clinic Managers during vacancies and any additional needs as designated by the Director of Operation. Train all new staff in the job functions to include utilization and documentation in CPRS. Nursing staff will be trained in proper triage process, ordering within nursing scope, and case management duties
Develop and maintain an RN/Case Manager and PACT team training guide
Follow up with PACT staff as a resource to effectively help fulfill the PACT team duties
CBOC operating hours are Mon - Fri 8:00am to 4:30pm.
Manage day to day clinic operations, ensuring clinical quality expectations are met
Compliance oversight, including Office of Inspector General, Joint Commission, Environment of Care, lab compliance, and other related items
Manage the clinic’s financial performance, review and manage clinic expenses
Take an active role in enrollment growth and retention
Assists practitioners with patient care and practices specialized nursing skills in patient assessment
Triage patients identifying acute patient needs and high acuity patients, identifying Veterans at risk for uncoordinated care, and mobilizing a care team for optimal patient outcomes
Provide patient and family education focused on self-management, prevention, and wellness
Monitor discharge list and contact patients for follow up
Establish and maintain a warm, welcoming, and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met
JOB QUALIFICATIONS:
2 years of healthcare management experience (VA CBOC preferred)
Primary Care and/or chronic disease management experience
Knowledge of CPRS, RN Care Management, LPN, and MSA duties, as well as the VAMC policies and procedures in each region that a PCS CBOC is established
Ability to train staff in PACT duties
Ability to access and review VA data with respect to clinical quality metrics
Ability to develop and roll out improvement plans
Current and active RN License for the state in which the CBOC is located, required to get a license in every VA location PCS has a CBOC as well as credentialed.
Current BLS for providers certification issued by American Heart Association (AHA).
Strong computer skills, including EMR experience.
