Position Overview: As a Registered Nurse (RN) Case Manager at Angels Care, you will be responsible for coordinating all aspects of patient home care. You will collaborate with other medical professionals and provide high-quality care to patients. Your responsibilities include conducting admission visits in patient homes, assessing eligibility for home care services, and developing personalized care plans. Qualifications: Graduate of an accredited nursing program (diploma, Associate, or Baccalaureate). Current state license as a Registered Nurse. Valid state Driver's License. Minimum of one year of nursing experience (two years preferred). Current CPR certification and Hepatitis consent/declination documentation. Reliable transportation with up-to-date auto liability insurance.
