No more than 1 preventable CMV accident in the last 2 years; no major CMV accidents in the last 5 years. No more than 2 moving violations in the last 2 years. No drug-related convictions in the past 5 years, or 10 years if it occurred while holding a CDL. No positive or refused tests in the last 10 years while holding a CDL-A/CLP.
