Responsibilities
Conduct assessments to determine the social, emotional, and environmental needs of clients.
Develop and implement care plans tailored to the individual needs of clients.
Provide counseling and support to clients and their families.
Collaborate with healthcare professionals to ensure comprehensive care.
Advocate for clients and connect them with community resources and services.
Maintain accurate and timely documentation of services provided.
Requirements
Master's degree in Social Work from an accredited institution.
Valid and current social work license in the state of Florida.
Previous experience in a healthcare or home care setting is preferred.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Flexibility to work on an as-needed basis, adapting to the varying needs of clients.
Valid driver’s license with reliable vehicle and auto insurance
