Responsibilities
As a Nurse Practitioner, you will conduct annual risk assessments either in a residential setting or via telehealth, adhering to the scope of practice and under the guidance of a Collaborative Physician.
Essential Duties and Responsibilities
Conduct comprehensive in-home health risk assessments to identify both active and chronic disease conditions, including physical, mental, and social needs present during the visit.
Gather patient history, perform examinations, and make diagnostic determinations.
Document patient visits in accordance with NCQA standards.
Take vital signs as necessary.
Manage case referrals and communicate with primary care providers as needed.
Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnosis and care.
Adhere to OSHA Universal Precautions in all clinical duties.
Maintaining patient confidentiality is paramount.
Attend required meetings and contribute to committees as requested.
Engage in professional development activities to maintain licenses and affiliations.

Qualifications
Required Knowledge, Skills, and Experience
Active and unrestricted Nurse Practitioner license in designated states
Board certification from ANCC, AANP, or NCCPA
Current CPR Certification
Enrollment in Medicare/Medicaid
Valid driver's license with a good driving record
Exceptional EHR proficiency
Preferred Knowledge, Skills, and Experience
Experience or training in geriatrics
Strong teamwork abilities and effective working relationships with patients and medical staff
Conditions of this role to be aware of:
Adaptability to diverse weather conditions and various home environments
Full range of motion, including lifting patients and manual dexterity
Regular lifting/carrying of items weighing up to 50 pounds
Ability to travel up to 150 miles daily in urban and rural settings
