In this role, you will play a crucial part in assessing, coordinating, and implementing patient care throughout the entire episode of care, with a strong focus on facilitating a smooth transition towards discharge. Your responsibilities will include: Assessing the needs of assigned patients. Coordinating and directing the patient care delivered by the caregiving team. Ensuring that all patient care is executed with compassion and competence. Acting as a liaison between patients, referring facilities, and hospital staff.
