Patient Services (Health Benefits) Position Summary: The Patient Services role is essential to provide Access Community Health Centers (Access) patients with comprehensive clinical care and ensure better health outcomes. This position collaborates with other clinical staff to assess patient needs and coordinate services. The main function of the Patient Services role is to provide case management to at-risk patients and/or prenatal patients and coordinate necessary follow-up care. This position is also responsible for tracking and reporting activities related to case management and referrals. Health Benefits: Helps patients with applications for Medical Assistance and the Affordable Care Act and administers Access's sliding discount program.
