Facility: GSS MN Brainerd Wdld Apt Location: 200 Buffalo Hills Ln W, Brainerd, MN 56401, USA Shift: Varies Job Schedule: Part time Weekly Hours: 16.00 Salary Range: $38 - $51 Department Details Work 16-24 hours per week, primarily during the day. Includes on-call rotation. Earn an additional $1.25/hour for weekend shifts and another $1.25/hour for hours worked between 6pm-6am. Job Summary As an RN Case Manager, you will be essential in coordinating the health care journey of residents, from pre-admission to post-discharge. You will integrate care to improve resident flow and enhance communication across teams, promoting early interventions and evaluating outcomes based on clinical and financial data. Your role will involve both structured and unstructured health care environments where you will apply independent nursing judgment. Your responsibilities include: Interviewing potential residents and assessing compliance with regulations. Creating tailored service plans and conducting ongoing evaluations. Communicating effectively with residents, families, and the healthcare team. Monitoring patient responses and controlling healthcare variances. Being fiscally responsible and knowledgeable about service reimbursements. You will need to demonstrate: Independence, assertiveness, and critical thinking skills. Clear documentation and problem-solving abilities. Knowledge of teaching principles adapted to age groups. Flexibility and organizational skills to establish priorities. Qualifications You should possess: A Bachelor's degree in Nursing (strongly preferred). A graduate from an accredited nursing program. A minimum of two years of clinical experience (preferred). An unencumbered RN license in the working state or multistate licensure if applicable. Required specific competencies and certifications.
