Position Summary
Your key responsibilities will include providing short-term counseling and crisis management for patients and families coping with illness, lifestyle changes, or financial concerns related to hospitalization. You will collaborate with healthcare professionals to create effective discharge plans and connect patients with necessary resources, ensuring a smooth transition out of the hospital and into their homes or other care settings.
Key Responsibilities:
Conduct psychosocial assessments and provide short-term counseling to patients and families within 24 hours of request.
Assist in discharge planning by identifying resources and documenting assessments in our electronic systems.
Screen patients for discharge needs within 48 hours of admission.
Develop comprehensive discharge plans in partnership with healthcare teams and family members, ensuring continuity of care.
Maintain strong relationships with community agencies and healthcare providers to facilitate referrals and resource connections.
Communicate crucial discharge information timely and accurately to ensure smooth transitions for patients.
Participate in discharge appeal processes and assist case managers with relevant information.
Qualifications:
To qualify for this role, you must have a Master’s degree in Social Work from an accredited program and be licensed by Tennessee Health Related Boards; LCSW is preferred. Previous experience in a hospital or clinical setting is required. Strong written and verbal communication skills are essential, along with proficiency in computer systems including Meditech and Allscripts.
