Position Purpose
Provides administrative leadership, management, and direction for Skilled Nursing. Has the authority and responsibility for taking all reasonable steps to monitor quality care provided, adherence to Annandale's philosophies, goals, and administrative principles with understanding and knowledge of current legal implications. Cooperates with Administration, all Annandale Directors and staff, and the Medical Director for appropriate and smooth integration of shared functions and responsibilities in the provision of healthcare services. Responsible for planning overall management of the SNF, for conducting the policies and procedures of Annandale Village, for the compliance with state requirements and for operating within the parameters of the planned financial budgets.
Required Qualifications
Graduate of an accredited college or university with a bachelor's degree in health care administration, Business Administration or other related field.
Must be certified as a Licensed Nursing Home Administrator in the State of Georgia and with six (6) years' experience in a health or aging related setting including two (2) years in leadership or supervisory position evidence of interpersonal communication skills necessary to interact effectively with the Board of Trustees, administration, Annandale managers and staff, physicians, residents and guardians and the members of the community; five years of healthcare related management experience.
Must be at least 18 years of age, have a valid Class C Georgia Driver's License, proof of vehicle insurance and meet company policy regarding MVR requirements.
Must meet state requirements regarding a GBI and FBI criminal history record check which requires fingerprinting.
Must undergo and pass a drug screening and screening for Tuberculosis.
Major Duties and Responsibilities
ALC Operational Leadership: Assume overall leadership for the Assisted Living Community (ALC) by directing strategic initiatives, enhancing operational efficiency, and driving improved financial oversight, including reducing reliance on casual labor.
Mentorship & Development: Provide dedicated 1:1 coaching to the ALC Administrator, focusing on strategic, big-picture thinking; financial management; reimbursement rate analysis; and navigating a non-profit structure.
Benchmarking & Best Practices: Collaborate with cross-functional teams and benchmark against industry standards by facilitating visits to other ALCs, ensuring the integration of innovative practices and resource optimization strategies.
Provides leadership for all staff of the facilities and participates in meetings as appropriate e.g., Leadership, Programs and Services, Skilled Nursing staff meeting, etc. Assist with approval of all admissions through participation with admissions committee.
Coordinates and integrates services both inside and outside of Skilled Nursing
Cooperates with the board, administration, and the medical director in determining the philosophy, objectives, policies, and standards of care for the facility and facilitating their implementation.
Establishes annual objectives, goals, plans and budget for the facility through collaboration with all corporate departments.
Participates in local, state, and national professional affiliations deemed necessary to accomplish the goals for the Facilities and Attends all required and assigned trainings and meetings. Performs other duties that may be in necessary in the best interest of Annandale at Suwanee Inc. To meet the personal service and care needs of the Villagers.
Recommends and implements policies and procedures that guide, support, and develop the facility services.
Recommends to senior management enough qualified staff and adequate supplies and equipment to provide quality care. Monitor staff performance to make sure that care and services are safely delivered in accordance with state requirements and Community policies and procedures.
Assign duties to personnel consistent with their position, abilities, training, and experience and in compliance with state requirements.
Establishes and maintains a mechanism to assure appropriate licensure of all professional licensed staff.
Continually assesses the performance and the services provided by staff assigned to the facility and assures quality of care and follow established safety procedures when performing tasks and/or working with equipment and/or Villagers.
Maintains quality control programs, as appropriate to the facility and Responsible for maintaining an effective Quality Assurance Performance Improvement Plan. Ensured and effective infection control program is being maintained.
Orients in-service training, continuing education and development to all staff assigned to the facility. Coordinate the orientation of fresh staff to the Skilled Nursing and Assisted Living and completes Orientation Checklist and determines readiness for independent work. Requiring all Staff members to complete initial and continuing education in compliance with state requirements and Community policies. Facilitates staff education as appropriate to ensure on-going competency.
Knowledge of state and federal regulations pertaining to Skilled Nursing
Whenever possible, remains at the Community during emergency situations. Whenever possible, remains available by phone when not in the community.
Ensures that all Care Plans are completed in a timely manner and meetings with families/caregivers are scheduled appropriately.
Acts as the contact person for the Skilled Nursing audits. Facilitates any corrective action plans necessary.
Ensures submission of required notifications and reports to DCH timely.
Evaluates Villager complaints and incident reports to determine the appropriate plan of care, communicates with physicians and other healthcare providers as necessary to ensure timely treatment and problems resolution.
Demonstrates knowledge of and competency in protecting the rights, dignity and individuality of Villagers and others.
Complies with all Annandale policies, procedures, and dress code; is punctual and dependable in performance of his/her duties.
Performs other duties that may be in necessary in the best interest of Annandale at Suwanee Inc. to meet the personal service and care needs of the Villagers.

The employee frequently is required to sit, stand, and walk; climb or balance; stoop, kneel, crouch, bend, or crawl. Employee is required to lift up to 25 pounds and/or move up to 75 pounds (with assistance). Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Employee is required to transport (drive) and assist Villagers in and out of vehicles. Employee is required to perform CPR/First Aid.
