Accountable Care Organization 401K Retirement Plan Paid Orientation and Training We operate in 11 states A+ rated malpractice coverage with tail coverage No holidays, no hospital rounds Responsibilities As a Nurse Practitioner, you will conduct annual health risk assessments in either a residential setting or via telehealth, within your scope of practice as delegated by a Collaborative Physician. Essential Duties and Responsibilities Conduct comprehensive in-home health risk assessments to identify both active and chronic conditions, including physical, mental, and social needs. Take medical history, perform examinations, and determine diagnoses. Provide thorough documentation of each patient visit in accordance with NCQA standards. Measure and record vital signs as necessary. Coordinate case management referrals and communicate with primary care providers (PCPs). Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnosis and care. Adhere to OSHA Universal Precautions while performing clinical duties. Maintain patient confidentiality at all times. Attend required meetings, in-services, and participate in committees as requested. Engage in ongoing professional development to maintain licenses and affiliations. Qualifications Required Knowledge, Skills, and Experience Active/unrestricted nurse practitioner license in applicable states. Board certification from ANCC, AANP, or NCCPA. Current CPR certification. Enrollment in Medicare/Medicaid. Valid driver's license with a good driving record. Proficient EHR skills. Preferred Knowledge, Skills and Experience Experience or training in geriatric care. Strong teamwork skills and ability to maintain positive working relationships with colleagues and patients. Conditions of this Role Ability to adapt to various weather conditions and home environments. Full range of motion to handle and lift patients as needed. Frequent lifting and carrying of items weighing up to 50 pounds. Ability to travel up to 150 miles daily in urban and rural settings. Compensation Earn between USD $85.00 to USD $145.00 per visit. Individual compensation packages reflect various factors such as skills, experience, and qualifications.
