We are seeking a skilled and compassionate Nurse Case Manager to join our team.

As a Hospice RN, you will be responsible for delivering high-quality end-of-life care to patients and their families.

You will work closely with the interdisciplinary team to assess patient needs, develop and implement individualized plans of care, and provide ongoing evaluation and support.

Your expertise and professionalism will play a vital role in ensuring the best possible outcomes for our patients and their families.

• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify and address patient physical, psychosocial, and environmental needs through ongoing assessment and re-evaluation.
• Develop and coordinate individualized plans of care that meet the unique needs of each patient.
• Provide education and support to patients and their families on self-care techniques and managing symptoms.
• Maintain accurate and detailed clinical records of patient care and progress.

• A minimum of one year of clinical experience as a Registered Nurse.
• CURRENT RN licensure in the state of practice.
• CURRENT CPR Certification.
• A valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
