Provide therapeutic interventions, including exercises and modalities, to help residents achieve their goals. Monitor progress, adjust treatment plans as needed, and provide education to residents and their families. Collaborate with an interdisciplinary team to deliver exceptional patient-centered care. Document assessments, treatments, and progress accurately and in compliance with facility and regulatory guidelines. Required Skills and Qualifications: Current Physical Therapist (PT) license. Graduate of an accredited Physical Therapy program. Experience in a Skilled Nursing Facility (preferred but not required). Strong communication and interpersonal skills.
