About the Organization Bluegrass Community Health Center is dedicated to contributing to the lives of patients through the compassionate provision of comprehensive primary healthcare. BCHC was originally established in 2001, with a federal 330 health center grant to serve local farm workers. In 2007, additional grant funding awards allowed an expansion to serve the greater community, including low-income families, the homeless, the uninsured, and Medicaid and KCHIP recipients.

Position LPN Category Nursing Rate of Pay Description
Minimum Education Required:
* Graduate of an accredited Nursing program
* Associate's degree in Nursing required; Bachelor's degree preferred for RN candidates
* Practical Nursing diploma or associate degree for LPN candidates
Certification/Licensure:
* Current Kentucky LPN or RN license in good standing
Minimum Experience Preferred:
* One year or more of experience in a clinical setting
* Two years of experience preferred for both LPN and RN candidates
Equivalencies:
* None
Required Knowledge and Skills:
* Proficient in nursing skills relevant to licensure level, including taking vital signs, administering medications, and assisting with patient care plans
* Ability to follow multi-step procedures and protocols
* Accurate and timely documentation in the Electronic Medical Record (EMR)
* Strong understanding of patient confidentiality and healthcare regulations
* Basic computer proficiency, including data entry and word processing
* Customer service orientation

Job Responsibilities (RN or LPN)
Provide competent and compassionate nursing care to patients in accordance with licensure scope (LPN or RN), using the nursing process under the direction of a provider or supervising RN (as applicable). Responsibilities include patient assessment, education, coordination, and support of clinical operations.
Patient Care:
* Deliver patient-centered care using evidence-based practices
* Administer prescribed medications, vaccines, and treatments within scope of practice
* Support the Refugee Health Screening process using culturally appropriate communication
* Conduct nurse encounters, triage, and patient education
* Obtain relevant patient medical information through tools like UK Portal and KASPER
Clinic Flow & Team-Based Care:
* Coordinate and support clinical flow to enhance efficiency and patient satisfaction
* Assist with daily clinic operations, including schedule management and staffing coordination
* Collaborate as part of a team practicing Patient-Centered Medical Home (PCMH) principles
* Serve as a liaison to leadership regarding staffing or operational concerns
Documentation & Compliance:
* Accurately document patient encounters in EMR
* Participate in quality improvement and compliance initiatives
* Attend required meetings and professional development trainings
* Adhere to all BCHC policies, laws, and clinical standards
Outreach & Support:
* Provide backup support to colleagues across the clinic as needed
* Participate in outreach events and health fairs
* Support ordering of supplies and equipment maintenance reporting
