Job Details Home Health Case Manager - LPN REQUIRED
Current State Issued Licensed Practical Nurse (LPN) license Current First Aid and BLS certifications are required prior to hire. Acceptable vendors for certification are American Heart Association and Red Cross. Personal transportation, valid state issued driver's license, good driving record and auto insurance as required by law. High School Diploma or GED PREFERRED 3+ yrs as LPN in a clinic or long term care setting with geriatrics experience Knowledge of medications used in the treatment of geriatrics We are looking for a healthcare professional who is ready to provide exceptional patient care in this permanent position.
