ALL APPLICANTS MUST HAVE LPN professional license in Massachusetts 1 year Traveler and 1 year Home Care experience. Ability to travel to cases as assigned. Job Summary: Provide quality nursing care to patients in various settings in the community. In collaboration with the RN case manager to provide skilled nursing visits. Utilizing thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.
