Responsibilities: Instructs and advises clients/residents on daily living skills in one-on-one or group instruction. Observes and monitors client behavior. Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems. Cares for individuals and families during periods of incapacitation, family disruption or convalescence, providing companionship, personal care, and help in adjusting to new lifestyles. Performs housekeeping duties, such as cleaning, washing clothes and dishes, and running errands. Plans, shops for, and prepares meals, including special diets, and assists families in planning, shopping for, and preparing nutritious meals. Assists with the development of an individual service/treatment plan for each assigned consumer/student. Participates in case reviews, consulting with the team to evaluate the client's needs and plan for continuing services. Prepares and maintains records of client progress and services performed, reporting changes in client condition to manager or supervisor. Provides transportation for clients/residents to appointments and activities. Arranges for needed medical care and provide needed services to assure that medical care is obtained. Assists in the presentation of educational group lectures related to appropriate topics. Occasionally, this position may be required to cover Day centers.
Technical Competencies: Ability to observe and monitor participants’ behavior. Ability to assist and supervise participants with daily living activities. Knowledge of agency confidentiality policies. Knowledge of individualized plans. Ability to develop individualized plans. Knowledge of the documentation process. Ability to build relationships with community employers. Knowledge of pertinent state, federal and local laws, rules and regulations. Ability to serve as a liaison between all interested parties. Ability to serve as a member of the treatment team to develop participants’ goals and objectives. Knowledge of program area. Ability to prepare area-specific reports. Knowledge of available resources. Ability to provide training and/or orientation services to participants. Ability to communicate both orally and in writing. Ability to investigate any concerns or complaints and develop a resolution. Ability to complete evaluations to determine the functional level of participants. Knowledge of various recreational activities. Ability to plan recreational activities for participants. Ability to participate in supportive guidance, staff supervision sessions, program planning meetings, consumer staffing and service training. Knowledge of agency safety policies and procedures. Ability to operate state vehicles following all safety laws and procedures. Ability to complete all daily housekeeping chores. Ability to identify and report facility repairs and equipment problems. Conflict resolution skills. Ability to complete participant orientation sessions. Ability to identify disruptive behavior. Ability to train participants in personal hygiene and other daily living activities. Ability to recognize contributions and celebrate accomplishments. Ability to conduct individual and group training sessions for participants. Knowledge of agency-accepted disciplinary techniques. Knowledge of agency-accepted group/interview techniques. Ability to develop teaching aids. Knowledge of case management procedures.

Qualifications: High School diploma or GED required. A CNA professional is not required but is helpful. One year of Development Disabilities experience. Previous supervisory experience preferred. Provide 2 professional references. Valid driver’s license.
Career Development Criteria: Complete agency-specific required training and certifications. Training in case management. Continuing education courses related to the job. Attending seminars. Group facilitation training. Conflict resolution training. Assessment training. Obtain any relevant certifications.
