Job Description:

Provide skilled PT services to adult and geriatric patients in their homes

Perform evaluations, develop care plans, and deliver therapeutic interventions

Accurately document services using EMR system: Wellsky (Kinnser)

Educate patients and caregivers to improve safety, mobility, and function

Collaborate with nurses and therapists for patient-centered outcomes

Requirements:

Active Texas Physical Therapist License

EMR Experience – Wellsky/Kinnser is a Plus

Home Health Experience is a plus

Current CPR Certification

Reliable Transportation, Valid Driver’s License, and Auto Insurance
