Northwest Kidney Centers seeks a full-time day shift Home Program Nurse Care Manager (Home Dialysis Training Nurse) to support the Federal Way, Fife and Kent Kidney Centers in peritoneal dialysis and hemodialysis. Shift: Full-time, day shift (0800-1630), Mon-Fri. Can be either 4 x 10s or 5 x 8s Purpose
The Home Program Nurse Care Manager (HPNCM) is accountable for the delivery and documentation of care of patients dialyzing at NKC and in their home. The Care Manager shall use a collaborative practice model focusing on outcomes to enhance the quality of patient care through effective and efficient management and monitoring of patient care programs. This includes comprehensive assessments, planning care, directing and evaluating the patients home environment and clinical results, education and training of patients and their helpers, as well as services and programs that enhance patient care. The nurse accepts and follows the leadership from the designated Charge Nurse, supervisor, or manager. Required qualifications Equivalent of Associate or Baccalaureate degree with nursing major
Current registered nursing license in the state of Washington
One year of previous nursing experience.
Preferred qualifications Previous dialysis experience

Paid Time Off includes personal time off and 8 Holidays. PTO will accrue bi-weekly and will be pro-rated based on the FTE. The accrual rate will be 7.39 hours each bi-weekly pay period for staff.
