Home Health Aide (HHA) Coordinator – Job Overview As a Part-Time HHA Coordinator (LPN) , you will support Case Managers and collaborate closely with our on-site physician during clinic days to ensure coordinated, high-quality care for our clients. This role combines clinical insight with administrative oversight—organizing, training, and supporting our Physician on clinic days. This will be a Part-time, in office role, functioning on Monday-Wednesday, with possibility of Thursday in the future. Key Responsibilities Collaborate directly with the on-site physician during clinic days to assess care needs and adjust services accordingly.
Ensure all documentation is accurate and up-to-date.
Maintain communication between HHAs, Case Managers, and clients.
Address and resolve scheduling issues promptly.
Deliver care kits and necessary supplies to clients.
Qualifications Valid LPN license in the state of TN.
Strong communication and organizational skills. Experience in a clinic setting, home health, case management Proficiency in Microsoft Office Suite.
Ability to pass a background check and drug screening.
Professional appearance and demeanor.
Additional Information This role may require travel between client locations using your personal vehicle.
Competitive pay is offered (specific range not provided).
This position does not include supervisory responsibilities.
