Position Overview:

• Location: San Antonio, Texas

• Schedule: Full-time, Monday Friday (5 days/week)

• Setting: Skilled nursing facilities (post-acute care)

• Employment Type: W2, benefits-eligible

What You'll Do:

• Provide primary care to adult patients in post-acute settings

• Manage chronic conditions, coordinate care, and support patient goals

• Collaborate with interdisciplinary teams and facility staff

• Document care using EMR tools, with dedicated training and support

• Contribute to quality improvement initiatives and patient safety programs

Ideal Candidate:

• Board certified in Internal Medicine or Family Medicine

• Active Texas medical license and DEA

• Post-acute care experience is a plus, but not required

• Strong interpersonal and clinical skills

• Interest in leadership opportunities and professional development
