Position Overview:

A skilled nursing and rehabilitation center in central Vermont is seeking a Family Medicine, Internal Medicine, or Geriatric Medicine physician to provide compassionate care to long-term care and subacute patients. The ideal candidate will have experience in post-acute or geriatric settings, but all board-certified candidates will be considered. This assignment begins October 8, 2025, and runs through the end of the year, with potential to extend.

Assignment Highlights:

Setting: Skilled nursing and rehabilitation center

Start Date: October 8, 2025

Duration: Through December 31, 2025 (with extension possible)

Schedule Options:

Monday Friday, 8:00 AM 5:00 PM preferred

Alternate schedules (Mon Thurs or Tues Fri) may be accommodated

Call: Yes, negotiable

Patient Census: 8 12 patients per day

Admissions

Discharges

Acute evaluations

30-day evaluations

EMR: PointClickCare (nursing home), Epic (hospital system)

Facility Type: Long-term care & subacute rehab

Team: Works alongside MDs and APPs; locum may consult with APPs but not supervise

Skills & Experience:

Board-certified in Family Medicine, Internal Medicine, or Geriatrics

Geriatric fellowship preferred but not required

Active BLS certification

Comfort with long-term care documentation and interdisciplinary collaboration

Pay: Top % nationally for geriatrics, family, and internal medicine locum roles
