Carthage Center is hiring a Director of Social Work (DSW) in Carthage, NY. \n\n\n DUTIES: \n \n Directs, establishes & plans the overall policies/goals for the Social Services Dept \n Responsible for assessments, care planning & helping residents adjust \n Supervision of Department Staff \n Discharge Planning at start of admission \n Occasional Community Outreach \n Familiar with a variety of concepts, practices & procedures \n A wide degree of creativity and latitude is expected \n Relies on extensive experience & judgment to plan and accomplish goals \n \n REQUIREMENTS: \n \n 2+ years of Management Experience in LTC settings \n LSW – MSW \n Knowledge of applicable State & Federal guidelines \n Exceptional interpersonal & leadership skills \n \n\n About us: \n Carthage Center for Rehabilitation and Nursing is a 90-bed rehabilitation and skilled nursing facility located on the outskirts of the historic village of Carthage.
