GENERAL SUMMARY
Manages and monitors assigned caseload of female clients participating in the St. Matthew's House Recovery Program (Justin's Place). Builds healthy, godly relationships with clients through mentoring and example. Develops program plans/goals and evaluates client's progress by conducting routine meetings with clients. Serves as an advocate for clients in order to obtain services-both within St. Matthew's House and through community resources-that will enable them to function independently. St. Matthew's House is a faith based 501(c)(3) charitable organization that exists to transform lives in a spiritual environment that is both compassionate and disciplined.
ESSENTIAL LEADERSHIP TASKS Challenges daily the clients in the program to be women who honor and walk with God in integrity and truth Performs client processes needed in the following areas: medical, financial and personal Helps develop program features that assist in the building of healthy relationship with clients Develops case plans for clients; instructs clients in completing necessary paperwork, including goals or budget forms; completes orientation checklist form and obtains release of information forms when appropriate Meets regularly with clients to discuss and evaluate their progress; prepares accurate and up-to-date records documenting same Prepares and maintains case notes dealing with extenuating circumstances relating to client and/or client issues; ensures the accuracy and completeness of same Helps plan, coordinate and facilitate life management classes to assist clients in obtaining skills that will enable them to cope with their environment; obtains and utilizes volunteers and workshop presenters as needed Checks periodically to ensure client's living quarters are properly maintained Performs random drug screens on clients that are on their caseload In conjunction with the Recovery Management Specialist, refers clients to other agencies based on client's needs and in accordance with their program plan Completes discharge of clients with accurate information Periodically attends community meetings with representatives of other agencies to keep them informed of any changes to the St. Matthew's House recovery program and updates any changes pertaining to the agencies' services (these meetings would also include AA, NA, Celebrate Recovery, etc.) Meets with immediate supervisor bi-weekly to provide updates on the status of client's progress Works closely with Recovery Management Specialists and when needed helps assist with class lecture/facilitation Utilizes other team members to address issues when assistance is needed with case management Be able to utilize all 12-core functions of Addictions Counseling
Requirements
Education : High School diploma required, Bachelor's/Master's Degree in
Psychology/Addiction Studies preferred. CAC/CAP desired.
Experience : A minimum of one years' experience working in clinical/addiction field.
Confidentiality : Works with highly confidential information.
Mental/Visual Demands : Flow of work requires normal mental and visual capabilities.
Physical Demand : Must be able to stand for a minimum of 4 hours at a time and life at
least 25 pounds. Must be able to bend or kneel to the ground.
Working Conditions : Normal office environment.
