We have an exciting opportunity for a high-performing individual to join our team of motivated and talented staff as a *Case Manager*, working with individuals in early recovery from Substance Use Disorders. Personal experience with recovery is crucial to this position because this is a peer to peer recovery service position.

*We have an opening for one full time or two part time Case Managers*. Hours are Monday through Friday, 11 am - 7 pm; required to cover core hours for case management work. Flexibility in work location is required. We have multiple locations in Woodstock and Crystal Lake - all within 20 minutes of each other. Must be comfortable with Case Management for both men and women.

*Responsibilities:*

* Assess client needs upon intake * Create individualized Action Plans consisting of goals specific to each client * Schedule and meet with clients weekly to evaluate progress towards Action Plan goal achievement and track progress electronically in our Client Management System * Must be comfortable with working in a male halfway house and with male clients in recovery from SUDs * Connect clients to community resources as needed * Administer drug tests and breathalyzer tests as needed * Assist clients with finding and maintaining employment * Create individualized Behavior Contracts to correct chronic behavior problems with clients * Attend regular staff meetings * Other duties assigned as needed * The typical case load will be between 12-18 clients at any given time, and will report directly to the Director of Housing.

*Qualifications:*

* Must be 18 or over * Must have proof of a valid IL driver's license and vehicle insurance with a good driving record * Bachelor's Degree in a Human Services or related field preferred * Must be familiar with the 12-step recovery method and/or 12-step facilitation therapy * Must be able to pass a background check
