We are excited to announce an opening for a full-time Behavioral Health Care Manager to join our dedicated team! This position is essential for providing care management to our patients in need, and offers an opportunity to make a significant impact in the community. Sign-On Bonus: $1500 (Terms and Conditions Apply) This is a grant-funded position under the Certified Community Behavioral Health Clinic (CCBHC). Position Overview: The Behavioral Health Care Manager will play a key role in connecting patients with vital health services, including medical, dental, behavioral health, and substance abuse programs. Your advocacy and support will help patients navigate their care journey through outreach, referrals, and community resource assistance. Key Responsibilities: Conduct outreach and intake using necessary forms and assessment tools, documenting in eCW and designated software. Collaborate with a multidisciplinary team to create and review client care plans and treatment goals. Facilitate referrals to medical and behavioral health services as guided by provider recommendations. Assist patients in accessing community and social services such as transportation, housing, and recovery programs. Engage clients monthly to assess ongoing needs and promote continuity of care. Coordinate with hospitals and healthcare teams to manage transitions in care, focusing on medication reconciliation. Educate patients to clarify provider instructions and enhance their understanding of health resources. Make field visits to ensure comprehensive care coordination for patients. Participate in community events supporting the CCBHC mission and services. Qualifications: Associate degree required; Bachelor’s degree in Social Work, Counseling, Psychology, or a related field preferred. A minimum of 2 years experience in social services is preferred.
